Click on the "Ear" ellipse, and drag it onto your unicorn's head.  Right now it looks more like a Teddy Bear Ear, and that just won't do, so we have to make some changes.  Because the ear is so small, you may want to zoom in once so you can better see what you're doing.

  Click on "Node Edit" in the Tool Options Box.  You should now see that the ear is invisible, except for the lines and nodes around it.

Click on the node at the top, you should see two ends of an arrow appear, drag each of the ends down, so that the top of your ear is pointy, like this:

Now, click on the node at the bottom, then right click your mouse, and a dialog box will appear.  Go to Edit, then Break.  You will see that you now have two nodes.  Move the left node to the left Edge of the Head, and the right node near the center.  Keep in mind that you can use those arrows here too, and change the shape to what you like. Choose Ctrl + Q to quit node editing. Here's mine:

Now that you have the ear just the way you like it, we need to add it to the other side of the unicorn's head.  Go to Edit (either at the top on your toolbar, or right click) and then choose copy.  Now choose Edit, Paste as new Vector Selection.  There's your Ear on the other side, but it's backwards.  To fix this, Click and drag the node at the center of the left side to the outer edge of your image, like this:

Now move it back onto the unicorn's head, like so:
